Please include a copy of, or provide confirmation of the following information:

› Description of approval process in parish
› Names and dated signatures of those persons with trusteeship of the property to be encumbered showing their consent to this indebtedness (may be parish Trustees, diocesan Trustees, or the Bishop).
› Architectural plans
› Governmental approvals
› Historic building, area or adjacent to either?
› Membership growth projections
› Parish membership
› Area population – census data attached? Check with county/city records or seek assistance from Percept information through diocesan deployment office.
› Above calculated at + / - 10%
› Any new church plants in area?
› Average attendance last five years
› Description of why growth is justified as given
› What does + / - range mean to parish ability to bear debt load?
› Stewardship history and projected growth rate

For the prior five years:

› Number of pledges made
› Number of pledges received
› Percent of pledged income not received
› Average pledge
› History of pledging to the Diocese. Must meet minimum pledge as stated in current diocesan policies in order to receive approval.

Also include:

› Operating budgets for prior three years
› Fund-raising report for project and timing of expected receipts
› When is the money coming in?
› Pledge assumptions properly discounted?
› Proposed borrowing and repayment plan
› Bank data, including proposed loan documents or pre-qualification documents
› All Diocesan Missionary Society (DMS) correspondence included
› Detailed spending plan by phase, including cost overruns
