On this page you will find links to the most commonly requested forms for use by Diocese of Virginia clergy, whether canonically resident, under license, serving parochially or serving in another capacity. These forms are not interactive and must be downloaded and printed. If you have any questions, or if a particular form is not available, please contact the Bishop's Office.

Note: To be relicensed, your Sexual Misconduct Prevention Training (both the child and adult workshops) must be current within 10 years.  If you’re uncertain when you last took the training, please contact the bishop's clerk at clerk@thediocese.net. Due to changes in diocesan policy, we will not be relicensing clergy without documentation of current training. Click here for information on retaking the training. Clergy who have previously received training are eligible to take the abbreviated trainings.

Please note: After July 1, 2011, those who are applying for licensure as Catechists or Eucharistic Visitors will be required to complete a short course in the Prevention of Sexual Misconduct.  Worship Leaders and Pastoral Leaders will be required to receive the same training for the Prevention of Sexual Misconduct that is required for clergy.  A list of course offerings can be found here.
